I use first. Like others here, my last name is always mispronounced (and thats when they even bother to try and say it). I am fortunate though that my first name is somewhat unique as well, so I always hear when I am called.
I agree with using the Mr. and Mrs. I think its a sign of respect. Kids should use it when addressing adults unless they are told otherwise - except with teachers. Teachers should always be addressed by the surname, in my opinion.
Adults should use the Mr. and Mrs. when meeting somebody new, unless indicated otherwise, and when in a formal setting (for example, in a banking environment) until indicated otherwise. Again, it's a sign of respect. Again, its my opinion.
